Free 75 yo Home Health Assessment
Health Assessments are a fantastic opportunity to provide preventative care to our elderly population, available for patients over 75 billed directly to Medicare offering our elderly patients the opportunity to obtain community services that will allow them to stay at home longer.

Chronic Disease Management Clinics
Illnesses such as heart disease, asthma or diabetes no longer need to control our patient’s lives. It’s all about education & management. With the latest equipment to help monitor your condition these clinics offer the opportunity to have regular detailed health checks that provide education to patients as well as initiate Medicare subsidised visits to allied health members such as podiatrists, dietitians, physiotherapists and psychologists.

Free 40-49yo Health Checks
The government has identified that early detection of risk factors in patients could be the key to preventing the increases currently seen in diseases such as diabetes. Therefore there is a subsidised health check available to patients aged between 40 and 49 that will be BULK BILLED directly to Medicare – that means no cost to you, take advantage of your free health check today.

Vaccinations:
A very serious way to prevent disease. Our staff are trained to update and inform our patients of recommended vaccines so our patients aren’t at RISK. Helen, Aileen & Joanne run our free immunisation clinic Monday to Friday.

Phone Calls
The doctors prefer not to take calls during consultations unless the matter is extremely urgent. If reception staff cannot help with a matter a message can be left and the doctor will return your call.

After Hours
The practice provides 24 hour care for our patients with our afterhours medical assistance Home Doctor Service. The normal surgery number will direct you to this number if you call outside of hours or you can phone 137425. Other alternative after hours care can be obtained by attending the casualty department at Logan Hospital.

Results
Our practice will notify you if you need to return for results. Alternatively you may contact the practice 3 days after having the tests for feedback. Follow up consultations with regards to receiving test results will be bulk billed for a 2 week period from the date of initial consultation, for discussion regarding results ONLY. Consultations discussing other issues will be charged at standard fee.

Reminder System
Our Practice is committed to preventative care, therefore provides a reminder system. We may issue you with a reminder notice from time to time offering you preventative health services appropriate to your care. If you do not wish to be part of this system please let your doctor or reception know.

Medical Records
Your health record is confidential and it is the policy of this practice to maintain security of health information at all times. If you require transfer of your notes to another practice a small fee will be incurred as our practice posts ALL medical records via registered post. We will also require signed authority. Our practice is also willing to transfer records onto a storage device provided by the patient after a 3 item security check has been performed and written authority has been obtained.

Your Health Information
To ensure the best quality of care we require the collection of medical information. This information is confidential and is kept private and secure. To ensure we maintain information security our practice abides by the 10 National Privacy Principles which are available for your viewing at www.privacy.gov.au/heafth/index.htmI.

Feedback
We love positive feedback, but if you have a complaint we encourage you to put it in writing so it can be addressed in a formal & professional manner.
