75 yo Home Health Assessment
Health Assessments are a fantastic opportunity to provide preventative care to our elderly population. Doctors consulting at Park Ridge offer their patients aged over 75 the opportunity to obtain an at-home medical services that will allow them to live at home longer. Your Doctor will bill their fees for your home health assessment directly to Medicare.
Chronic Disease Management Clinics
Illnesses such as heart disease, asthma or diabetes no longer need to control your life. It’s all about education & management. With the latest equipment to help monitor your condition doctors we offer you the opportunity to have regular detailed health checks that provide education as well as initial Medicare subsidized visits to allied health members such as podiatrists, dietitians, physiotherapists and psychologists.
40-49yo Health Checks
The government has identified that early detection of risk factors in patients could be the key to preventing the increases currently seen in diseases such as diabetes. Therefore there is a subsidised health checks available to patients aged between 40 and 49.
Vaccinations:
A very serious way to prevent disease. Your Doctor is trained to update and inform you of recommended vaccines so you aren’t at RISK! The Vaccination clinic runs Monday to Saturday at Park Ridge. To book, phone reception on 3802 0899 or book on hotdoc.
Phone Calls
Your doctor prefers not to take calls during consultations unless the matter is extremely urgent. If reception staff cannot help you, a message can be left and your doctor will return your call.
After Hours
The Doctors consulting at Park Ridge provide 24 hour care for their patients with an afterhours medical assistance Home Doctor Service. The normal reception number will direct you to this number if you call outside of consultation hours or you can phone 137425. Other alternative after hours care can be obtained by attending the casualty department at Logan Hospital.
Results
Reception will notify you on behalf of your Doctor if you need to return for results. Alternatively you may contact reception 3 days after having the tests for feedback.
Non Attendance Policy Your doctors run very busy practices and regularly have to turn patients away or place them on cancellation lists due to their busy schedules. Your doctors ask that if you cannot attend your appointment, to provide at least 4 hours’ notice. If you fail to attend your appointment without notice full consultation fee will be payable without the ability to claim a rebate from Medicare.
Reminder System
Park Ridge Medical is committed to supporting preventative care and therefore facilitates a reminder system for the doctors they support. You may be issued with a reminder notice from time to time offering you preventative health services appropriate to your care.
Medical Records
Your health record is confidential and it is the policy of your Doctor to maintain security of health information at all times. If you require transfer of your notes to another practice a small fee will be incurred as your Doctor posts ALL medical records via registered post. Your Doctor will also require signed authority. Your Doctor is also willing to transfer records onto a storage device provided by you after a 3 item security check has been performed and written authority has been obtained.
Your Health Information
To ensure the best quality of care your Doctor will require the collection of medical information. This information is confidential and is kept private and secure. To ensure your Doctor maintains information security they abide by the 13 Australian Privacy Principles which are available for your viewing at www.privacy.gov.au/heafth/index.htmI.
